Topical Finasteride and Dutasteride: Do They Really Work?

In the world of hair loss treatments, the use of topical finasteride and dutasteride has sparked considerable interest. With my previous video on topical finasteride amassing over a million views across platforms, it's clear that many are curious about the effectiveness and safety of these topical 5-alpha reductase inhibitors (5ARIs).

In this blog post, we'll explore whether topical finasteride and dutasteride actually work, the difference between various formulations, and the latest innovations in this field. We'll also delve into the findings of two key studies conducted by independent labs at the University of Milan's pharmacology department.

What Are Topical 5ARIs?

Topical 5ARIs like finasteride and dutasteride work by inhibiting the enzyme 5-alpha reductase, which converts testosterone into dihydrotestosterone (DHT). Elevated levels of DHT are associated with hair loss, particularly in androgenetic alopecia (male pattern baldness). The topical application of these drugs is designed to target the hair follicles directly, reducing the risk of systemic side effects often seen with oral versions of these medications.

Do Topical 5ARIs Reduce Side Effects?

One of the major advantages of topical finasteride and dutasteride is the reduced risk of systemic side effects. These side effects, which are most commonly sexual in nature, include low libido, erectile dysfunction, and mood disturbances. Our previous analysis revealed that topical formulations significantly reduce plasma drug levels and have a less pronounced impact on total serum DHT compared to oral medications. This translates into fewer side effects for patients.

Are All Topical 5ARI Solutions Created Equal?

After extensive research, one product that stands out is developed by Xyon Health, co-founded by Dr. Simone Pimstone. This product features a patented silicone-based solvent known as the Syloxis system. This innovative delivery method creates a mesh that allows the drug to penetrate only to the level of the hair follicle, thereby minimizing systemic absorption.

The Science Behind the Syloxis System

In Vitro Study: Franz Cell Test

The first study we’ll look at is an in vitro trial that utilized a Franz cell test. This test measures drug absorption using human epithelium, comparing Xyon's Syloxis system to other popular topical solutions. The results were promising: the finasteride with the Syloxis system demonstrated the least absorption through the membrane, suggesting that it is less likely to cause systemic side effects.

In numbers, the Syloxis system showed about 0.6 micrograms of absorption, compared to 1.5 mcg and 4 mcg with other solutions, all at a 2.5% concentration.

In Vivo Study: Real-World Application

The second study involved 10 patients in an in vivo trial, measuring the systemic absorption of finasteride and dutasteride when applied topically using the Syloxis system. Baseline DHT levels and drug concentrations were established before treatment, and the results were compared to those of patients taking oral finasteride (1mg daily) or oral dutasteride (0.5mg daily).

The findings were significant:

  • Topical Finasteride: Plasma levels of finasteride in the topical group were about 0.75 ng/ml, compared to 9 ng/ml in the oral group—a 10x reduction in systemic absorption.

  • Topical Dutasteride: Plasma levels of dutasteride in the topical group were around 1 ng/ml, versus 27 ng/ml in the oral group—a 27x reduction in blood concentration.

These results indicate that the Syloxis system effectively reduces systemic absorption, thereby lowering the risk of side effects.

Potential Drawbacks and Criticisms

While the data is compelling, it's important to note some limitations. The in vivo study was conducted on only 10 patients, which is a small sample size. Additionally, the study only compared the Syloxis system to oral finasteride and dutasteride, without including other topical formulations.

Another consideration is availability—this product is currently only accessible in the USA and Canada, which limits its global reach. Lastly, the cost of this advanced formulation may be prohibitive for some patients.

Conclusion: A Promising Solution in Hair Loss Treatment

Despite these limitations, the Syloxis system by Xyon Health represents a significant advancement in topical hair loss treatments. The technology offers a potentially safer alternative to oral 5ARIs, with robust data supporting its efficacy in reducing systemic absorption and side effects. Notably, Xyon Health was founded by Dr. Victor Hasson, one of the most influential hair transplant surgeons globally, further solidifying the credibility of this product.

For those considering topical hair loss treatments, the Xyon Syloxis system appears to be a promising option. As always, consult with your healthcare provider to determine the best treatment plan for your needs.

Final Thoughts

While more studies with larger sample sizes would provide even stronger evidence, the current data on Xyon’s Syloxis system is compelling. The product is currently available in the USA and Canada, and while it may be on the higher end of the pricing scale, it offers a viable solution for patients looking to minimize side effects while maintaining efficacy in hair loss treatment.
